A Case Report on Treatment of Frequent Urination and Nocturia Due to Benign Prostatic Hyperplasia (BPH) with Cai-Hu-Based Decoction and Acupuncture Therapy

【摘要】

良性摄护腺增生(Benign Prostate Hyperplasia, BPH)好发於五十岁以上的男性,随着年龄增长,良性摄护腺增生的盛行率也逐渐上升。由於摄护腺位在膀胱出口下方,包覆着膀胱与尿道连接处,当摄护腺腺体组织随着年龄增长而增生时,肿大的组织会压迫尿道,进而出现频尿、夜尿、尿急、解尿不畅、尿流速慢等下泌尿道症状,造成日常生活的不便。此例为75岁男性,本身有睡眠呼吸中止症、脑中风、心肌梗塞病史,频尿已有十年,但近两年前开始症状加重,且夜尿两、三小时一行,因而影响睡眠品质。解尿时有解不尽感,白天解尿很慢,流速低,但尿量极少。诊断病机为「肝气失调,少阳枢机不利,气滞血瘀」,经医师针灸合并柴胡剂以枢转少阳,治疗数月後症状改善,追踪回诊数月後已可停用西药,且诸症平稳,未再发生相关症状。藉此病例报告提出不同治疗思路,供未来临床医师叁考运用。

【Summary】



Benign prostate hyperplasia (BPH) is a common prostate disease in men over the age of fifty, and the prevalence raises with age. Prostate gland is located below the bladder and encases the junction of bladder and urethra. The enlarging prostate gland compresses urethra as the glandular cells over-proliferate with age, leading to some symptoms of lower urinary tract such as frequent urination, nocturia, urgency, difficult urination and slow urination rate. These symptoms may disrupt daily life of patients. This is a 75-year-old man with medical history of obstructive sleep apnea, stroke, and myocardial infarction. He has suffered from frequent urination for ten years; however, the symptom has exacerbated in recent two years, accompanied with nocturia so that has disrupted sleep. He also complained his incomplete emptying of bladder, slow urination, and low urine volume. Considering his symptoms, "stagnation of liver, dysfunction of Shao-Yang, and blood stasis" were suspected as the primary impression. After several months of treatment with acupuncture and traditional Chinese medicine, which contained Cai Hu Shu Gan decoction, his symptoms improved so that held on Dutasteride and Desmopressin. This case report is presented to offer an alternative facet toward managing benign prostate hyperplasia.
