肌肉与关节对应取穴治疗中风急性後期患者肌肉痉挛之病例报告

Muscle and joint corresponding acupuncture for the treatment of muscle spasm in patients with stroke post-acute care

【摘要】

中风後的肌肉痉挛与张力增大是中医会诊及门诊极为常见的求诊原因，

主要成因为肌肉一直处於收缩的状态，而过强的肌肉张力不仅会产生疼痛、

紧绷与僵硬等不舒服的感觉，也会导致病人在日常生活中必要的行为活动

无法顺利完成。本病例报告为一中风後导致右侧偏瘫一个月之男性病患的中

医诊治过程。主要的症状为右半部偏瘫无力、肌肤麻木。医师经临床评估与

病因病机四要素的分析之後，决定采取疏经通络兼行气活血的治疗方法，并

尝试由肌肉与经穴对应的角度下手，针对挛缩或无力的肌肉来选取适合的穴

位。病人经一个月的治疗後，在巴氏量表及美国国家卫生院脑中风评估表

（National Institute of Health Stroke Scale, NIH Stroke Scale）上的项目均有明

显的进步与改善。本病例呈现与主流之专注於经络循行的取穴策略不同的治

疗方法，并对於中风後接受急性後期整合照护的患者取得一定的成效，供先

进同道叁酌讨论。

【Summary】

Muscle spasms and hypertonicity in patients with stroke are common in Chinese medicine clinics and consultations. Hypertonicity not only causes pain, tightness, and stiffness but decreases the mobility in stroke patients. This case reported the treatment in a stroke patient with right hemiplegia for a month. There were symptoms including right side muscle weakness and skin numbness. Based on activating qi and promoting blood circulation, we used acupuncture to stimulate muscles and acupuncture points that aim to adjust contracted or weak muscles. After a month of treatment, it showed a significant progression in this patient on the NIH stroke scale and Barthel scale. Our study provides a different aspect of treatment from the mainstream therapy which selects acupoints along the channel, and it reveals a significant progression in the patient that received post-acute integrated care after stroke.
