探讨《温病条辨》中口腔各部位乾燥之诊断分析

【摘要】
口乾之症状影响全球约四分之一人口,其症状可大大影响患者生活品质,在西医中以口乾症作命名,在中医典籍中最早从今本《黄帝内经》起即有对於口乾症状描述,於唐《千金翼方》中更分类出消渴篇章对於此症状及用药有更多了解,於明清时期口乾论述广泛出现於典籍中,其中在《温病条辨》中口乾、渴的症状开始大量出现於书中,《温病条辨》为清代医家吴鞠通撰写,其着作以三焦辨证为纲领,系统化统整出多种温热病类型,叙述温病的病因、分类、传变及预後。本文针对《温病条辨》文中针对口、唇、舌、咽等口腔各解剖部位的乾燥,作三焦、卫气营血及方剂分析与探讨,内容包含各条文中三焦归类、症状与方剂卫气营血及其他层次归纳分析、探讨上、中、下三焦病位及病程如何辨别,及口渴、渴不欲饮水层次区别,特殊邪气影响层次范围,最後归纳出面对温邪造成口乾时其临床诊断路径如何着手,提供未来医者在温热病范畴中口乾的临床诊断的想法与叁考。

【Summary】
Dryness in the mouth affects about a quarter of the world’s population, and the symptom significantly affect the quality of life of patients. Dryness in the mouth also called xerostomia in western medicine, and the symptom significantly influenced patient’s quality of life. The symptom of dry mouth has been mentioned in Huangdi Neijing first. There were some section which described emaciationthirst disease in Supplement to Thousand Ducat Formulas written in Tang dynsaty. The description of dry mouth and thirsty widely used in Ming and Qing dynasties, especially in the Wen-Bing-Tiao-Bian. Wen-Bing-Tiao-Bian, written by Wu Ju Tong at Qing dynasty, systemically integrates various kinds of warm-heat evils and describes the cause, classification, transmission and prognosis of the warm diseases. This article analyzes the dryness of the mouth, lips, tongue, pharynx, and discusses the relation of triple burner and Wei-Qi-Ying-Xue level of warm diseases at different oral parts. The content inherits the tradition of Wen-Bing-Tiao-Bian and also concludes the clinic thinking process of how to approach and diagnosis oral dryness by triple burner and Wei-Qi-Ying-Xue theory and we expect to provide different idea for clinical use.
