针灸治疗膀胱过动症之病例报告
A Case Report of Overactive Bladder
Treated with Acupuncture and Moxibustion

【摘要】
膀胱过动症为临床常见的疾病,其涵盖的范围非常广泛,包含急迫性尿失禁、混合性尿失禁以及一部分的应力性尿失禁。其定义为解尿急迫感、合并频尿及夜尿,有些人可能伴有急迫性尿失禁。其评估方式包含病史询问、理学检查与尿液检查,使用膀胱过动症调查问卷及排尿日志。可用於评估病患於治疗前後,症状及疗效的变化。其治疗包含一般卫教、行为治疗以及药物治疗,药物治疗通常选用作用於副交感神经药物,用於减少膀胱逼尿肌收缩,但可能伴随口乾舌燥、便秘等副作用。
本病例报告为24 岁膀胱过动症病患,因诊断膀胱过动症已九个月,期间也曾服用药物但效果并不理想。故至本院中医部寻求治疗。在接受针灸治疗後,各项症状也迅速获得良好控制,因国内期刊较少使用针灸治疗膀胱过动症的病例,故整理此医案作为针灸治疗膀胱过动症的临床叁考。

【Summary】
Overactive bladder is a clinically common disease that covers a wide range of conditions, including urge urinary incontinence, mixed urinary incontinence, and part of stress urinary incontinence. It is defined as urgency, combined with frequent urination and nocturia, and some may be associated with urge incontinence. The assessment include history taking, physical examination and urine examination, use of the overactive bladder questionnaire and voiding diary. It can be used to assess changes in symptoms and outcomes before and after treatment. The treatment consists of patient education, behavioral treatment, and pharmacologic management. Drug therapy is usually used as a parasympathetic drug to reduce bladder detrusor contraction, but may be accompanied by side effects such as dry mouth and constipation.
This case report is a 24-year-old overactive bladder patient who had been diagnosed of overactive bladder for nine months and received drugs, but the effect was not satisfactory. Therefore, he searched traditional Chinese medicine for help. After receiving acupuncture, the symptoms were quickly and well controlled. Because less acupuncture for overactive bladder was published in the journal, this case was arranged as a clinical reference for acupuncture of overactive bladder.
