中草药乳霜对湿疹患者的疗效评估及生活品质改善之研究

Evaluation of the Efficacy of Herbal Creams on Eczema Patients and Improvement of Quality of Life

【摘要】
目的:湿疹是一种常见的皮肤疾病,由遗传、环境、饮食不当及细菌感染等引发,表症为皮肤乾燥、搔痒、抓痕、脱屑及结痂,且易反覆发作,影响生活品质。西医用类固醇治疗,虽有效,但长期使用可能导致副作用。中医则透过祛风燥湿、清热解毒等药材治疗,虽疗效良好,但需长时间煎煮,配合度低。因此,开发中草药乳霜剂型治疗湿疹具有潜力。方法:本研究招募35位湿疹患者,撷取中医处方「苦叁洗汤」,结合左手香、小本七层塔等民间草药,制成乳霜剂型外用药。每日两次涂抹於患处,并在为期十二周的研究中进行六次面访及照片评估。评估指标包括皮肤生活品质量表(Dermatology Life Quality Index, DLQI)、异位性皮肤炎评估指标(SCORing Atopic Dermatitis, SCORAD)及湿疹面积暨严重度指数(Eczema Area and Severity Index, EASI)。结果:治疗前後,患者在DLQI、SCORAD和EASI指标上均有显着改善(p<0.001)。具体数据显示:DLQI从高度影响降至轻度影响(18.17→6.49);SCORAD从中度影响降至轻度影响(40.90→12.51);EASI指数由轻度上限降至轻度下限(4.97→1.46)。数据表明,改良後的中草药乳霜剂型在治疗湿疹方面不仅有效,还能提升患者生活品质。结论:本研究证实,中草药乳霜剂型治疗湿疹的有效性,兼具保湿及使用便利性,为湿疹提供患者新的治疗选择。未来若能针对重度湿疹患者进行相关研究,将进一步验证中药介入的有效性。

【Summary】

Objectives: Eczema is a common skin condition triggered by genetic, environmental, dietary factors, and bacterial infections, characterized by dry skin, itching, scratching, scaling, and crusting. It often recurs, impacting quality of life. While corticosteroids are effective treatments in Western medicine, long-term use may lead to side effects. Traditional Chinese medicine (TCM) employs herbs to dispel pathogenic wind, eliminate dampness, and clear heat-toxin, which are effective but require lengthy preparation, resulting in low compliance. Therefore, developing a herbal cream formulation for eczema treatment shows promise. Methods: This study recruited 35 eczema patients and utilized the TCM prescription "Kushen Decoction", combined with local herbs such as Plectranthus amboinicus and Salvia plebeia, to create a topical cream. Patients applied the cream twice daily for twelve weeks, with six follow-up visits for assessments. Evaluation metrics included the Dermatology Life Quality Index (DLQI), SCORing Atopic Dermatitis (SCORAD), and Eczema Area and Severity Index (EASI). Results: Significant improvements were observed in DLQI, SCORAD, and EASI scores post-treatment (p<0.001). Specifically, DLQI decreased from 18.17 to 6.49, SCORAD from 40.90 to 12.51, and EASI from 4.97 to 1.46. These results indicate that the improved herbal cream formulation is not only effective in treating eczema but also enhances patients’ quality of life. Conclusion: This study confirms the efficacy of the herbal cream formulation for eczema treatment, offering moisturizing benefits and convenience, thus providing a new therapeutic option for patients. Future research targeting severe eczema patients will further validate the effectiveness of TCM interventions.
