Scalp Acupuncture for the Treatment of Japanese Encephalitis

【摘要】
日本脑炎(Japanese Encephalitis,JE)是由日本脑炎病毒所引起的急性脑膜脑
炎,受损部位包含脑、脊髓以及脑膜,致死率约20%至30%,存活病例中约30%
至50%有神经性後遗症包括不正常肌张力、语言障碍、运动肌无力、脑神经及
锥体外系统异常的神经功能缺损。本篇以一名54岁男性患者,反覆发烧持续一
周,至花莲慈济医院就诊後,隔日仍有反覆发烧以及喉咙痛、流鼻水、咳嗽有
痰与头痛的情形,再次至急诊检查与治疗,入院後经卫生署疾病管制署确诊为
2023 年花莲首例日本脑炎,住院期间因纳差、下肢无力与反应迟缓而会诊中医
,出院後并於中医门诊持续治疗,针灸以百会、四神聪、足运感区、神庭、眉
冲、本神、头维、囟会为主,昏迷指数GCS由E4V1M4进步至E4V5M6,希
望本病案能提供日本脑炎之中医辅助治疗的临床叁考。

【Summary】
Japanese Encephalitis (JE) is an acute meningitis caused by the Japanese Encephalitis virus, primarily affecting the brain, spinal cord, and meninges. The mortality rate of JE is approximately 20% to 30%. Among survivors, about 30% to 50% suffer from neurological sequelae, including abnormal muscle tone, speech impairments, motor weakness, cranial nerve dysfunction, and extrapyramidal system abnormalities. This case study focuses on a 54-year-old male who presented with recurrent fever persisting for a week before seeking treatment at Hualien Tzu Chi Hospital. Despite initial medical intervention, the patient still continued to experience fever, sore throat, rhinorrhea, productive cough, and headache, prompting further emergency evaluation. The patient was subsequently diagnosed by Taiwan Centers for Disease Control as the first confirmed case of Japanese Encephalitis in Hualien in 2023. During his hospitalization, he was consulted by traditional Chinese medicine (TCM) specialist due to poor appetite and lower limb weakness. After discharge, the patient continued to receive TCM outpatient therapy, primarily involving acupuncture at Baihui(GV20), Sishencong(EX-HN1), Foot Motor Sensory Area, Shenting(GV24), Meichong(BL3), Benshen(GB13), Touwei(ST8), and Xinli(GV22). Notably, the patient’s Glasgow Coma Scale (GCS) score improved from E4V1M4 to E4V5M6. This case aims to provide valuable insights for clinical practice of Japanese Encephalitis with traditional Chinese medicine.
