针刺对脑中风患者血压心率之影响及失眠疗效初探
Effect of Acupuncture on Blood Pressure and Heart Rate in Patients with Stroke and Insomnia

【摘要】
目的:(1) 探讨针刺疗法对脑中风与非中风失眠患者的血压及心率影响如何?(2) 评估针刺疗法对失眠的疗效如何?方法:台北市立联合医院中兴院区於2014 年10 月至2016 年11 月间招募失眠受试者,分为脑中风及非中风两组,利用匹兹堡睡眠品质量表(PSQI) 及电子血压计量测受试者针刺前後的血压及心率,以探讨针刺疗法对失眠患者血压及心率的影响,并评估其疗效。
结果:共招募失眠受试者79 人,其中非中风组42 人,脑中风组37 人。无论是非中风或是脑中风失眠患者,接受针刺治疗後两组受试者的心率值皆降低,且睡眠品质亦获得改善。两组受试者的收缩压及舒张压平均值皆在正常范围内,但脉搏压则有偏高的现象,且脑中风组受试者历次的脉搏压皆显着高於非中风组。脑中风组针刺第3、4 次时收缩压及脉搏压有增加的情形,前者平均值增加6.2mmHg 及5.5mmHg,後者平均值增加4.3mmHg 及4.6mmHg。
结论:针刺疗法可降低失眠患者之心率,并改善其睡眠品质。失眠患者之脉搏压有偏高的现象,可能增加血管硬化的风险;而脑中风组之脉搏压高於非中风组,代表其血管硬化程度可能较非中风组严重。部分患者针刺後血压升高,可能是因个性紧张所导致的暂时性血压升高。针刺疗法基本上是一种安全、有效的治病方法。

【Summary】
Objective: To explore 1. Were there different effects of acupuncture on blood pressure and heart rate between stroke patients and non-stroke patients? 2.What were the physiological or pathological features of patients who had greater variability in blood pressure and heart rate after receiving acupuncture treatment ?
Methods: A trial was conducted from October 2014 to November 2016 in Zhong-Xing Branch, Taipei City Hospital, Taiwan. The participants were measured by using electronic sphygmomanometer to examine effects of acupuncture on the blood pressure and heart rate.
Results: A total of 79 participants including 42 of Group non-stroke insomnia patients and 37 of Group stroke insomnia patients were enrolled. The result showed that the heart rate of both groups decreased and their sleep quality improved after receiving acupuncture treatment. The systolic and diastolic blood pressure of the two groups were within the normal range. But the pulse pressure of the two groups were abnormal and the stroke group was significantly higher than insomnia group. The systolic blood pressure and pulse pressure of the stroke group might increase after receiving acupuncture treatment.
Conclusions: The decreased heart rate and improved sleep quality of both groups revealed the potential beneficial effects of acupuncture in treating insomnia. Insomnia may be related to arterial stiffness. The arterial stiffness of patients in the stroke group were more severe than in the insomnia group. Some patients had higher blood pressure after acupuncture may be due to personality stress. Acupuncture is basically a safe and effective treatment method.
