Review of Chinese Medicine Archives and Modern Studies about Atopic Dermatitis

【摘要】
异位性皮肤炎是一种慢性、反覆发作的皮肤疾病,时常从年轻时就开始侵犯身体,对於患者的生活品质、人际关系、家庭照护,都造成一定的负担,而目前的治疗方式仍以西医为主轴。虽然西医治疗效果不错,但也有许多患者担心西药所造成的副作用,因此转而求助於中医治疗,并希望藉由根本将异位性皮肤炎完全治愈,不再发作为治疗目标。
本回顾查找了古代典籍中描述接近於异位性皮肤炎的相关文献,了解中医是如何描述此种疾病,并试着将文献叙述与临床所见症状结合在一起,让古今医学的连结更加紧密。另外,也翻阅了近十年的相关研究报导,发现异位性皮肤炎患者的证型以「脾虚湿热」为最大宗,「血虚风燥」及「湿热内蕴」则分别占了第二及第三。在相关脏腑方面,以「脾」对整个疾病的影响最大,而外邪中则以「火、热」与「湿」分别名列一、二。
在整个文献的回顾过程中,我们希望从中寻找合适的治疗方向,希望不只能为临床治疗提供叁考,也能对未来的研究能有所贡献。

【Summary】
Atopic dermatitis (AD) is a chronic and relapsing disease which often strike a person from young ages and cause negative effects on life qualities, social networks and health care system. In our society, Western medicine is still the major therapy for AD patients to choose and the consequences are fine under most situations. However, considering the side effects of taking Western medicine for a period of time, some patients turn their eyes toward Chinese medicine for not only to alleviate those annoying symptoms but to cure AD through whole system adjustment.
In this review, we tried to realize the thinking process of Chinese medicine science through searching some of the archives and to establish a closer connection between traditional illustrations and modern symptoms. In the same time, we assembled some of modern studies in 10 years found out that the most seen pattern/syndrome is “spleen deficiency with dampness encumbrance”, which “blood deficiency and wind-dryness” at the second place and “spreading dampness-heat” takes the third. As the Viscera and Bowels part, “Spleen” plays the major role. Talking about External Pathogen, “Heat/Fire” affects most while “Dampness” is the second.
We hope to summarize a suitable therapy direction during this review process which not only to offer a reference for clinical work but to promote our future researches.
